The US Dept. of Transportation requires specialized training for Substance Abuse Professionals (SAP) conducting DOT compliant substance evaluations for employees in safety sensitive duties. In addition to substance abuse certification, DOT/SAP’s must complete 12 hours of specialized training every 3 years and pass a national exam specific to DOT guidelines. Camye Thibodaux meets or exceeds all Department of Transportation (DOT) criteria for Substance Abuse Professional (SAP) evaluations.
Evaluations are conducted by appointment. Recommendations are made by the counselor, based on the client's answers to a questionnaire. Reports are written to the client and others, including employers and DOT agencies, where applicable. When the client successfully completes the counselor's recommendations, a follow-up appointment is required by law. Documentation is collected from the client, a follow-up interview is conducted, and completion reports are written to the client and others. A schedule for continued drug testing is provided to the client's employer and DOT agency, as stipulated by law.
Fees for SAP services:
Initial Intake, Evaluation, and Reports- $300.00
Monthly Monitoring Fee - $100.00
Follow-up Appointment, Interview, and Reports - $300.00
Fees if Services Delivered Outside of Office - Per case, mileage plus expenses
